Boa tarde, tudo bem?
Preciso criar um novo formulário de prova para a residência médica, é possível criar um formulário com 4 provas diferentes, utilizando apenas 1 link?
Vou colocar os acessos diretos para as provas e também os de pré requisito mas queria utilizar apenas 1 link seleciona o nome e abre somente a prova que será realizada.

Now, coming back to your question, if I understood correctly, do you want to have 4 different forms but just want to use only 1 link? You cannot have multiple forms with the same link. As a workaround, you can instead create a main form and you can hyperlink the other forms so they can just click on it and will be redirected to it. If that works for you, here's how to do it:
1. Create a main form and add a Paragraph element to it.
2. Now go to your other forms and get the links. Let's do the link for Test 1 as an example. Go to your 1st form.
3. In Form Builder, in the orange navigation bar at the top of the screen, click on Publish.
4. Click on Copy Link.
5. Now let's go back to the Main Form.
6. Highlight Test 1 and click on the hyperlink icon on the upper right of the box.
7. Paste in the form link as the URL value and click Ok.
8. Click on Save on the paragraph element and that's it. Apply this to all of your tests.

Now, you can share the tests with the form fillers just by using the link of the Main Form and they can just choose which test they want to be redirected to.
