O Formulário em questão está reportando um erro de "campos incompletos" e que faltam informações de contato, porém, todas as informações foram preenchidas e mesmo assim ele informa esse erro.
Podem me ajudar com isso?

Or, if you'd rather have support in Portuguese, let us know and we can have them do that. But, keep in mind that you'd have to wait until they're available again.
Now, coming back to your question, I cloned your form, and I can easily replicate it. The issue seems related to the field 'Seus dados de contato', that you had on the card version layout of the form. Since you don't have any submissions yet in the form, you can delete the faulty field in the card version, if you no longer need it, to fix the issue. Let me walk you through it:
1. In Form Builder, click on the blue Roller Paint icon.
2. On the right panel, go to the Layout tab.
3. Then, click on Card Form to change layout.
Once you've switched to the card form layout, you can proceed with deleting the faulty field. Let me show you how to do it:
1. In Form Builder, select the Seus dados de contato field
2. Then, click on its Trash icon to remove.
Once you've successfully deleted the field, you can switch the form layout back to Classic Form.
