Thanks for reaching out to Jotform Support. Your account was suspended because you’ve got a form named WEB Family System Form - Hotel Settings and Management Chapeco Head Office that’s breaking our Terms of Use by collecting login credentials.
If you agree to follow our Terms of Use and let us delete the offending fields mentioned above, we can go ahead and lift the suspension on your account.
Once we hear back from you, we'll be able to move forward with this.
