Como criar uma condição de regra múltipla

Preciso criar um formulário com uma seria de perguntas, de acordo com as condições de periodicidade e do vistoriador, que poderia ter algumas perguntas iguais a outros vistoriadores, por exemplo:

You can add a dropdown field to your form with different inspector's name and can add the inspector part in already added conditions so that if all the rules of the conditions are matched the field will only then display. ​​Let me help you do it:

1. In the form builder, click on Add form elements and add a dropdown field.
2. Add Inspector name in the field.

Now you need to create a condition or edit you already created condition in a way that all rules of the conditios are matched. For example SELECIONE O CONDOMÍNIO is equal to Mensal and The Inspector is equal to 1 and all the rules are matched the field will only be visible then. Here is a screenshot of a condition as an example.

Agora, complementando, eu preciso criar este formulário baseado em uma lista de atividades de inspeção onde, quando escolher a PERIODICIDADE, aparecerá as ATIVIDADES separadas pelos SISTEMAS.

Estas atividades ficam em uma planilha de Excel, conforme figura abaixo:

É possível importar esses dados da planilha do Excel é transformar as ATIVIDADES em perguntas para serem respondidas?

Unfortunately, It is not possible to upload the data from the Excel sheet and turn them into questions. However, you can add the questions on your form and use Show/Hide Conditional Logic to display certain questions based on the answer.
