Boa noite,
estou utilizando um formulário em que há um campo de EMAIL, o qual é utilizado como ID Exclusiva. Exporto as respostas regularmente em MS-Excel e importo em minha plataforma de gestão de pacientes. Observei que o campo de email aceita endereços de email INVÁLIDOS, ou seja, erros de digitação neste campo impactam negativamente a identificação dos respondentes do formulário pois não tem cumprido a função de um campo de ID Exclusiva.
Por gentileza, solicito orientação sobre eventual abordagem para que eu possa utilizar algum item de resposta como ID Exclusiva.
Muito obrigado.
Nota: tentei CPF, mas não há máscara para tal dado

Thanks for reaching out to Jotform Support. Yes, you can restrict the same email to fill out your form multiple times. Let me show you how:
- On your My Forms page, hover your mouse over your form and click on Edit Form on the right side of the screen.
- In Form Builder, in the orange navigation bar at the top of the page, click on Settings and then click on Show More Options.
- In the Form Settings menu, scroll down and select Email in the Unique Field dropdown menu.

Many thanks for your reply.
That’s not quite what I need.
Repliers will interact with the form not only once, but multiple times. I need to avoid that the same person uses a different email or any other kind of Unique ID every time he/she responds. For example: I need to avoid that he/she types peter@example.com once and then pettter@example.com the other time.

Hi institutoparducci,
You may want to use our Spreadsheet to Form widget. This widget lets you upload a spreadsheet to your form and use the values to prepopulate your form. In your case, you can use the email address for the access code column. This will avoid them entering a typo to their email address. Let me walk you through this:
First, create a spreadsheet with the same column name as the field label in your form. Check out my screenshot below for preference:
Now, let's add the Spreadsheet to Form widget:- In Form Builder, click on the Add Form Element on the upper left side of the page.
- From the Widgets tab, search for Spreadsheet to Form.
- Click on the Wand icon of the widget.
- You can drag or drop the Excel file or click on the Upload file button and upload the file.
- Once the Excel file is uploaded, select an Access Code Column. You can use the email here.
- Click on the Update Widget button at the bottom of the page.
Next, let's set the email field as required. Then, we can pass the value from the Spreadsheet to Form widget to the email field using Update/Calculate Field condition. Let's do this:
- In Form Builder, click on the Gear icon of the Email field.
- From the General tab, toggle Required.
We can now set up the Update/Calculate Field condition:
- In Form Builder, click on Settings on the orange navigation bar at the top of the page.
- Select Conditions from the left side window.
- Select Update/Calculate Field.
- Set up the condition like the one below.
- Click on the Save button at the bottom of the page.
Here's how it works:
As you can see on my screencast above, there's a warning message that shows Unknown access code when I've entered a wrong email address.
