Formulário congelou na segunda página

Olá, já criei meu formulário, mas na última página, antes da página de agradecimento o sistema não passa, como se congelasse nesta 2ª página. Como corrigir

Now, coming back to your question, I can see two forms in your account that have multiple pages. I've cloned the Avaliação Clínica Laser form and was not able to replicate the issue. I was able to submit the form and got routed to this page:
