Widget Planilha para Formulário - Erro

Boa tarde!

Estou com um problema em todos os meus formulários que utilizam esse Widget mencionado


A opção "Auto Complete Fields" apareceu (substituindo o nome do botão que era diferente) e ele está impossibilitado de clicar, ou seja, preencho o campo mas ele não aplica a ação de preencher os campos "Nome" e "Email", o botão inclusive apresenta uma questão de que ele não aparenta estar sendo acionado ao clique.

Now, coming back to your question, I've checked out the screenshot you shared with us. I've cleared your form caches; your forms are now refreshed. Can you please open the Spreadsheet to form widget and the click on the update button? Let me show you how to do it:

1. Open your form in Form Builder, and in the orange navigation bar at the top of the screen, click on the Spreadsheet to Form widget's magic wand icon.
2. Select Update widget button on the right side of the panel. Close the widget's settings. That's it.

When I cloned and tested your form Entrada de Estoque - Bragança Paulista by adding my testing Excel sheet, I could not replicate an issue. Let me share the screencast below:
