Now, coming back to your question, Your account was suspended because you’ve got a form named INSTALLATION CHECKLIST that was tagged as Phishing. You should also refrain from using "password" as your label to avoid being flagged by our phishing detector. Kindly change it to something similar, like an "access code". Other than that, no other kinds of passwords or user credentials are allowed to be collected.
We understand that this might not be the case with your form, but we had to act according to the general rule to be in compliance with our Terms of Use.
We've gone ahead and deleted the offending field and reactivated your account. You should be able to log in now.
