I have checked the form, and it does not have any custom URL. For your future reference, keep in mind that If you have set up custom URLs, once the form is transferred, the custom URLs of the form will no longer work. You’ll need to re-create them again. You can learn to do that here with our guide about how to create a custom URL for a form.
