autorizo a regular meu folrmulário apagando o campo que deve ser excluído mas preciso da minha conta de volta.

Now, coming back to your question, your account was suspended because you’ve got a form named Formulário that’s breaking our Terms of Use by collecting login credentials.
I've deleted the offending field and reactivated your account. You should be able to log in now.
