Erro no anexo de imagem no formulário

Após o anexo da imagem dentro do campo "Anexar Evidência" no envio do formulário apresenta um erro que está na imagem abaixo anexada.

Now, coming back to your question, I believe you're referring to the LAUDO TÉCNICO DE COBERTURA DE SINAL form, correct? If so, I cloned your form and tested it to see if I could replicate the issue, but everything was working properly. Check out the screencast below to see my results:

I've cleared out the cache of your forms to fix some minor bugs. Can you try again and see how it goes? If the issue persists, can you take a screenshot of what’s happening and send it to us? We’ve got a guide here that’ll show you how to do that. Also, is the same issue happening no matter what image you try to attach to the evidence field?
