Criamos um formulário, porém na visualização Web ele está deconfigurado. Não aparece todos os campos para preencher.
VERSÃO WEB. Tudo bem!
VERSÃO MOBILE: Não aparece todos os campos
Como podemos resolver isso?

Now, coming back to your question, When i tested your form in a mobile device, the asme thing happened to me. The form is not entirely seen. Can you try to embed the form using its iframe code. Let me show you how:
- In Form Builder, click on Publish.
- Select Embed.
- Then choose iframe.
- Click on the Copy Code button.
Give it a try and let us know how it goes.
