Como crio um formulário com resultado de média?

Olá, bom dia. Espero que esteja bem.

Gostaria de saber como faço um formulário de 5 alternativas de escolha única para cada pergunta, e que cada alternativa da pergunta tivesse um valor de 1 a 5, e no final somar todas as perguntas e exibir a media com um resultado para o usuário.

I understand that you would like to create a form with 5 single choice questions and the options to have a value from 1 to 5, and at the end add up all the questions and display the average with a result for the user. You can do it by using the Single Choice element and a Form Calculation widget. Let me show you a simple demo form.

1. In Form Builder, click on the Add Form Element button.

2. Add a Single Choice element to your form and Configure the questions and values to your preference.

3. Click on the Widgets tab and select Form Calculation widget.

4. Click on the Magic Wand Icon to open the Widget Settings and set the recommended Calculation Values below and once done click on the Save button.
