Now, coming back to your question, I'm afraid you'll need to reach out to the Google Support team if you need help with managing Google forms. A quick Google search as well, and it will give you this guide on how to customize your Google forms.

If you actually meant Jotform, though, let me show you how to add an upload button:

1. Open your form in Form Builder and click on the Add Form Elements menu on the left side of the screen.
2. Under the Basic tab, scroll down to the Field Upload element and add it to your form. That’s it.
