Não estou conseguindo fazer as alterações do formulário conforme circulado em anexo.

Poderia me ajudar como posso fazer as alterações?

Now, coming back to your question, are you referring to editing form submissions in Jotform inbox? If yes, then you can edit it by opening the form prefilled with the submission’s data, changing the prefilled values, and then submitting the form. Here's how to do it:

1. In your Jotform Inbox, click on the submission that you want to edit.
2. Click on the Three Dots icon on the right side of the Comments button.

Keep in mind that if this isn't your Jotform Inbox, but you want to edit submissions, the owner of the Inbox has to change the Permissions in Access Settings to Collaborator.
