How to Choose a Folder on Clickup and Send Submission Data there via the Integration

Boa tarde, criei um formulário para enviar a clientes. Como posso fazer com que as respostas sejam transferidas para o clickup para uma pasta? Ou seja, que no clickup automaticamente seja criado um documento com as perguntas e respostas
