Suspended account due to phishing username and password

The form https://form.jotform.com/240065717942054 was found suspicious of phishing activity because you're collecting user credentials.

Gathering these types of data violates our Terms of Use.

Asking for an access code to show/hide fields within your form conditionally is okay. However, you should refrain from using "password" as your label to avoid being flagged by our phishing detector. Kindly change it to something similar to an "access code". Other than that, no other kinds of passwords or user credentials are allowed to be collected.

We will only lift the suspension on your account if you agree with our terms and allow us to delete the offending field mentioned above.
