Create a graph for another form

Tenho um formulário sobre RELATÓRIO DE ATIVIDADES, gostaria de saber se é possível criar um gráfico com os dados inseridos separadamente.

Exemplo:

Como no print o gráfico de Quadro pessoal e quadro de treinamentos não traz separado as informações de cada setor.
Pois cada coordenador é responsável pelo seu preenchimento e o que preciso é gerar um gráfico para cada clínica para realizar uma comparação se as atividades estão sendo executadas.
é possível gerar o relatório?

Now, coming back to your question, to clarify, are trying to create a graph based on the data from another form? If so, you will need to first download your submissions as CSV, then combine them using the Combine Submissions integration. If that’s not exactly what you’re looking for, can you explain a bit more so I can help you better?

As informações deste formulário é preenchida por pessoas diferentes, ou seja, nossos enfermeiros coordenadores que pertence a um respectivo setor (Clínica Médica, Clínica Cirúrgica, UTI Adulto, UTI Neo...), o que preciso é criar um gráfico das respostas separadas não unir as respostas em um único gráfico, é possível?

Aqui como na foto tenho a tabela preenchida por cada coordenador para seu setor. Gostaria de criar um relatório mensal de cada setor, exemplo mês de Abril um gráfico e outro grafico para o mês de Maio da Clínica Médica e realizar contrareferência se as atividades passadas pela Gerência estão sendo realizadas.

Now, coming back to your question, you can create a Digest Email Report to create monthly graphs for a selected form. Let me walk you through it:

1. On your My Form Page, click on the Downward Arrow icon on the top-left side of the page and select My Reports.

2. Click on the blue Create Report button, located on the top left side.

3. Click on Digest Emails, select Monthly under Date setting, and then click on Next.

4. Select a form and then click on Next.
