Configurable List Widget: Is it possible to add image on the check list?

Por favor teria como eu usar um checklist conforme a imagem abaixo com a possibilidade de ter um campo de imagem em cada linha?

Esse exemplo abaixo, eu consigo configurar um campo para adicionar imagem?

Now, coming back to your question, Based on my understanding, you are trying to sell using your form and you want to add an image to each line in the configurable list widget. Unfortunately, there is no option to add images to the configurable list widget. I've gone ahead and escalated your request to our developers. When or if it's implemented depends on their workload, how viable it is, and how many other users request it. If there are any updates, we’ll circle back to this thread and let you know. 

In addition, I would suggest using our product list field if you would like to sell and adding images to the options or selections. You can check out more details on how to set up a product list at this link.

Na verdade não vou fazer venda, preciso montar um cheklist que eu tenha a opção de acrescentar fotos!
