How to allow the respondents to upload a file to my form?

Preciso de um campo que a preencher a pessoa envie uma imagem, mas o campo "imagem" disponível não me permite que a pessoa que está preenchendo o formulário envie, mas sim que está o criando.

Como faço isso?

Now, coming back to your question, you can add the File Upload field to your form. This element will allow the form respondents to upload images or other files to your form. Here's a screenshot of the form element that I am suggesting:
