Now, coming back to your question, I checked our account and saw that one of your forms is in the Archive section of your account. If that's the form that you're looking for, please unarchive your form to show it in the My Forms section of your account. Let me show you how:

1. Open your My Forms page.
2. In the menu on the left side of the page, scroll down and click on Archive.
3. Choose the archived form that you want to unarchive and then click on More.
4. In the dropdown menu, just scroll down and click on Unarchive and that's it. You're done.

Now, that form will be in your list of forms on your My Forms page.
