    Now, coming back to your question, It appears that you are referring to a form you filled and submitted. If you are not quite familiar with Jotform, Jotform is used to create online forms. We have a lot of users using Jotform to complete order forms, survey forms, quiz forms, and more.

    Please get in touch with the form's owner by responding to the email you received after submitting the form. They will be able to help you with your request. We host the forms they produced, therefore, we advise contacting them directly instead.
