Gostaria de anexar um botão para imprimir no fim do meu formulário, mas que o formato da impressão seja como o modelo de pdf criado.

Now, coming back to your question, Yes, you can set it up so anyone filling out your form can download their answers. Let me show you how it is done:

1. In Form Builder, click on the Settings tab and click on Thank You Page on the left menu.
2. In the Thank You Page menu, click on Action Buttons, click on Download PDF, select Default PDF in the PDFs to be Downloaded dropdown menu and click on Create.

Now, anyone filling out your form will be able to download their answers:

You can also add the Download PDF option to the Autoresponder Email. Let me show you how:

1. In Form Builder, click on the Settings tab and click on Emails in the menu on the left.
2. Hover your mouse over your Autoresponder Email and click on the Pencil icon.
3. Click on the bottom cell in the table and then select Insert Row After.
4. Now just click on the empty cell, click on Form Fields, and click on PDF Link.

Now, anyone filling out your form will get an email with a link to download the submission PDF:
