Now, let me help you with your question. Are you referring to form 232403370681046? If so, this form is deleted but rather it is disabled. You can set the status of your form to Enabled in 3 easy steps. Let me show you how:

1. Open your form in Form Builder.
2. In the orange navigation bar at the top of the page, click on Settings.
3. In the Form Settings window, set the Form Status to Enabled. That's it, you're done.
