How to collect multiple data in a single field in the form?

Preciso colher dados de contas dos meus clientes com os seguintes dados: Tipo de Conta - Corrente ou poupança; Agencia - com digito verificador separado ex: 0758-7; Conta tambem com digito verifcador separado; E operação. Qual elemento eu poderia usar para colher essas informações num unico campo?
