Now, let me help you with your question. If I understand correctly, you'd like to create a form that you can also fill out? If so, you can definitely build a form. I checked your Jotform accounts, and you appear to have multiple forms already created. To fill out your own form, toggle the Preview Form or click the open a new window icon next to your form URL in the upper left corner of your screen. Check out the screencast below to see my results:
