Thanks for patiently waiting. Let me show you how you can customize your form URL. You can change the form URL, so it has a name in place of the numbers, but it will also include your username:

1. Open your form in Form Builder, and in the orange navigation bar at the top of the page, click on Publish.
2. Then, click on the Pencil icon to update the URL.
3. Now, just click on Apply and you’re done.

If you want to use your own domain in your form URL, then you will need to upgrade to the Jotform Enterprise plan.
