Now, let me help you with your question. I cloned your form to test it, and I easily replicated the issue. There is an easy fix for this, we just need to add a Submit button on the form. Let me show you how:

• In Form Builder, click on the + Add Form Element on the top left.
• Scroll down and add Submit to your form by dragging it to the bottom part of the form.

You can also change the Submit button alignment. It's really easy, just click on the gear icon, and under the General tab, click Right on the Button Alignment option.
