Hi Roger,
I have reviewed your account, and it seems that you have created the form Documentação com Contador Anterior (RH) again. Please take note that this form contains the word Password which automatically trigger our system to suspend your account. I have deleted the offending form and reinstated back your account. You can now login back to your account.
