Possibilidade de criar testes de temperamento e arquétipos e questões com 4 opções de resposta

Olá, não conhecia esta plataforma e achei no google. Quero saber se é possível criar com ela um teste de temperamento (colerico, sanguineo, fleumático, melancolico) e um teste de arquetipos.

Tentei criar e tive muita dificuldade.

E preciso de um formulário que tenha 4 opções de respostas, mas so achei modelos com 3 opções.

Você pode me dar essa informação/orientação?
