Bom dia, tudo bem?
Estou criando um formulário para agendamento de MATRÍCULAS aqui em nosso Hospital, gostaria de saber se existe uma forma de que quando clicar no link aparecer uma tela com senha antes de abrir o formulário para preenchimento.
Exemplo: clico no link e aparece na tela pedindo os 4 números finais do meu CPF, pois temos que ter certeza de quem vai estar acessando, por o link do edital estar aberto em nosso site. Ser algo restrito apenas para os aprovados.

Thanks for reaching out to Jotform Support. Unfortunately, our Portuguese Support agents are busy helping other Jotform users at the moment. I'll try to help you in English using Google Translate, but you can reply in whichever language you feel comfortable using. Or, if you'd rather have support in Portuguese, let us know and we can have them do that. But, keep in mind that you'd have to wait until they're available again.
Now, let me help you with your question. Please note that you can password-protect your form. This will require users to enter a code before being able to view the form. Let me show you how to do it:
- In Form Builder, click on Settings at the top of the page.
- Click the Show More Options button.
- Scroll down and enable the Password Protection.
- Set up your password.

Mas, a senha configurada é padrão para todo mundo?
Porque tenho a lista dos nomes dos aprovados ai tem como cada um digitar a sua senha (exemplo: 6 digitos finais do CPF) e ai sim abrir a tela para preenchimento?

Thanks for getting back to us. Unfortunately, passwords may only be used to protect submissions; they cannot be used to fill out the form. As a workaround, you can use conditional logic to display the field based on the user's answer. Let me show you how:
- On your My Forms page, hover your mouse over your form and click on Edit Form.
- In Form Builder, click on the Add Form Element menu and drag-drop the fields you want to use as follow-up questions (Short Text fields are used in this example).
- Click on the Type a Question text and change it to the question you want to use.
Now, let's set up the conditions:
- In Form Builder, in the orange navigation bar at the top of the page, click on Settings.
- Click on Conditions on the left of the page.
- Now, click on Add a New Condition and then click on Show/Hide Field.
Set the conditions like you see below:

Thanks for getting back to us. We're here to help you. Let me walk you through it:
- First, add a short text field to your form. You can also change the field label depending on your needs.
- Now we need to add a condition that will show all the form fields once we enter the given password.
- In Form Builder, in the orange navigation bar at the top of the page, click on Settings.
- Click on Conditions on the left of the page.
- Now, click on Add a New Condition and then click on Show/Hide Field.
Set the conditions like you see below:
- IF: CPF number
- STATE: Is Equal To
- VALUE: 123456 (You can add conditions as much you want to with your required numbers.)
- DO: Show Multiple
- FIELD: Select all fields, including the submit button.
