Thanks for sharing the additional information. I checked further and wasn't able to figure out how you are getting to this page on the screenshot shared by you above. I tested the Online Shop on your app 233563374119660, and it seems to be working as expected for me. Take a look at the screencast below to see my results:
As an additional measure, I've cleared the form cache for you to avoid any temporary cache issues, so can you try again to see if this makes any difference?
