Now, let me help you with your question. If I understood what you’re asking, do you want to add logo in your form? If so, it is really easy to add a logo on your form. Let me show you how:

1. In the Form Builder, click on Add/Edit Welcome Page at the top of your form.
2. Select Click to add a logo. Upload your picture in it.
