nosso formulário de cadastro sp não funciona no tablet está funcionando apenas no computador, por gentileza, o que devemos fazer porque nossos clientes preenchem os dados no tablet e é apenas este formulario que esta dando problema

Now, let me help you with your question. I'm sorry you're having trouble with this. I cloned your form CADASTRO -SP and tried opening it on an Android tablet, Samsung Galaxy Tab S7 and I was able to do so. Check out my screenshot below:
I was also able to open it using a mobile phone, Samsung S23 Ultra.
The same thing happened when I also checked your actual form URL:
It would help us to understand what’s happening if you could take a screenshot and send it to us. We have a guide here explaining how to do that.
Can you also please provide with us the following details so we can have a better idea of the issue?
- Mobile Phone/Tablet's Brand and Model
- Mobile Phone/Tablet's Version
- If using the Jotform Mobile App, please provide the app's version
- If not, please provide the browser used when opening the form from the tablet

or prorblema não é acessar o formulario o problema é que a cliente preenche todo o formulario CADASTRO - SP e quando a cliente clica em finalizar simplesmente não finaliza ou seja fica na tela sem conseguir salvar o cadastro que a cliente acabou de digitar...nos ajude por genitleza

Thanks for getting back to us. Thank you for the clarification that the issue happens upon submission. I understand that the form is not successfully submitted and nothing happens. I tried submitting my cloned form using a Samsung Tablet, and I was able to successfully submit it. The only minor issue I can see is that the Thank You page looks like it wasn't loaded properly, which causes you to think that the form was not submitted successfully. Here's a sample screencast below:
The Thank You page is not displayed properly. Only a white line is displayed instead. But as you can see on the tab's label it says Thank You. This would indicate that the form is successfully submitted
I did another test submission in my cloned form via the web browser and a mobile phone, and I got the expected Thank You page.

Can you please confirm if you're having the same issue? This looks like an issue related to responsiveness, and the submissions made from the tablet are indeed saved. You can check on the Submission Table after a submission is made from the tablet to make sure that the form was successfully submitted. You can check the submissions in 3 ways. Let me show you how:
1. Select the form in your My Forms page and you can see that there is a Submissions button at the top of the page.
2. You can also click on the Submissions right under your form.
3. Lastly, when you click on More and then select Submissions under Data in the droplist.
You can also confirm if the submission was successfully submitted when you check the inbox of the set Notification Email recipient. Here's a sample notification email that I received after I submitted the form using a tablet.

só para sinalizar, o formulário CADASTRO -SP voltou a funcionar sozinho e não fizemos nada de diferente para ele voltar a funcionar, mas voltou a funcionar. Obrigada!
