Dzień dobry.
Klient rejestrujący się poprzez formularz, po wypełnieniu i przejściu dalej otrzymuje komunikat jak w załączonym zrzucie ekranu. Dodam że chodzi o formularz: "zgłoszenie na egzamin". Proszę o wsparcie.

Now, let me help you with your question. When I cloned and tested your form, the same thing happened for me. After investigating it a bit, I found out that PayU integration was causing the problem. Can you try to remove the integration and re-enter your credentials?
