Now, let me help you with your question. Are you referring to the Notification email in your form? If you can share the link to your form, I can check it out and see what’s going on. I've checked your account but didn't see any forms. Let me show you where to find your form URL:

1. Open your form in Form Builder, and in the orange navigation bar at the top of the screen, click on Publish.
2. Under Quick Share, click on Copy Link to copy your form URL.
