Dzień dobry,
Odnotowaliśmy problem z działaniem formularza. Przesyłam link do formularza oraz screen z występującą przypadłością.Bardzo proszę o pomoc w rozwiązaniu problemu. Nasz formularz aktualnie nie działa i musieliśmy zejść do epoki papieru i długopisu.

Just to clarify, is this happening only to one user? Are your other forms working correctly? Do you know if your users are using mobile devices when submitting the form? Also, is your form embedded on a webpage? If yes, please share the link of the webpage too.
