Getting an error when submitting form

Witam serdecznie,

Od kilku dni napotykam się z błędem formularza, przy wysyłaniu wiadomości.

Wyświetla się komunikat:

Error on query: answersDB<br>Error returned:MySQL server has gone away<hr>

Proszę o pomoc w rozwiązaniu problemu.

Can you try it again and see how it goes? If you run into the same issue again, let us know and we'll do some more testing to see what's going on.

Właśnie przetestowałem kilka razy formularz (z różnymi obrazkami jako załączniki) i za jednym razem zadziałał, ale później znowu nie zadziałał z tym samym obrazkiem, ani z innymi. Ten sam błąd cały czas.

Czy mógłbyś to sprawdzić jeszcze raz?

Can you try to re-embed your form using the iFrame method? If you're still having the same issue, can we get permission to test submit on the form embedded from your website?
