I checked your account and found that it was suspended because of collecting passwords on the "Email Signup Form".
Please note that collecting Login or User Credentials such as Passwords, Security Q&A and Phrases, and PIN codes are not allowed regardless of the type of account. These fall against Jotform's Terms of Use. In order for us to reinstate your account, you would have to agree with our terms and allow us to delete the offending fields and any associated submission data with it.
Let us know if you agree with our terms and the deletion of your form data and we'll be able to help you further.
