How can I activate suspended account?

Now, coming back to your question, I've checked your account and I can see that it was auto-suspended by our anti-phishing system. This is because the form was collecting Username and Password on the form. Please note, it is in violation of our Terms of Use to collect any kind of password and username on the form. Since there were no submissions on the form I've gone ahead and remove the form from your account and activated the account. You should be able to log in now.

Thanks, it worked. I created form probably just to check and i didn't realize there is request for name and password, just got some theme i guess. Thanks again.
