Now, let me help you with your question. If I understood what you’re asking, you are referring to this form and it is not working. I cloned your form and tested it to see if I could replicate the issue, but everything was working properly. Check out the screencast below to see my results:
Can you explain a bit more about the issue, so I can get a better idea of what's happening? Is there any error message or does the form just does not load at all? It would help us to understand what’s happening if you could take a screenshot of any error and send it to us. We have a guide here explaining how to do that. I have also cleared your form cache in case it may help. Can you try it again and see how it goes? If you run into the same issue again, let us know, and we'll do some more testing to see what's going on.

Thanks for getting back to us. I checked your account, and it's not currently over quota. I've cleared your form cache to refresh the form. This should resolve the issue. Although caches aren't cleared automatically, it's easy to do it manually. Let me show you how:
- On your My Forms page, hover your mouse on your Profile Avatar icon.
- In the window that opens, click on Settings.
- In the Settings menu, scroll down to the bottom of the page and click on Clear Cache. That's it.
