I've checked your form and tried to open on the Preview Mode to replicate the issue but didn't encounter any issues. The form is loading and working successfully on my end.
The issue you encountered could be temporary. Also, I cleared your form cache. Can you try again on another browser, and if the issue persists, can you share a screenshot or screencast to help us understand better? To do that, you can follow this guide, or if you want to share a screen recording, you can use Loom.Give it a try and let us know if you need any help.

Hi Raf,
Thanks for getting back to us. I'm suspecting that this is a regional issue based on users or most likely ISP related. Do you have another service provider such as a mobile data service and see if you are able to access the form there?
As soon as we hear back from you we'll be able to narrow down the issues and look for a solution.

Can you send us a screenshot of the Console Report of your browser? It would help us determine what is causing the problem.

I Know, on some places and networks it works, other not.
We have also customers where it workts, other not.
So i hope you can tell what causes the local problem some time en some times not.

Thanks for getting back to us. I cleared your form's cache again to remove the piled-up cache on your forms that could be contributing to this problem. Can you try it again and let us know if the problem is the same? If it is. Please send us a screenshot of your Browser's Console Report. We have a guide on how you can get it here.
Once we hear back from you, we can move forward with a solution.

I found the problem. Your .com site is nog legally compliant with EU rules.
So problem is 100% jotform problem.
Poor support.

According to your screenshot, you are accessing your form at "form.jotform.com". Unfortunately, some Internet Service Providers in Belgium have blocked it. Until then, we've introduced an alternative domain for our users from Belgium. The Backend Team recommends our users to utilize https://form.jotformeu.com.
We will contact these ISPs from our side but it would be helpful if you can also contact your provider for IP blacklist removal.
We really appreciate your patience and understanding while we’re looking into this.
