    For us to lift the suspension on your account, we’ll need you to send us some more details about your business and the purpose of your form or any proof that you have that it’s legit.

     1. The URL where the form is used.

    2. Provide any document that proves your affiliation to a legitimate institution.

    3. The website or social media page to where to find your service or business

    Once we hear back from you, we'll be able to help you with this.
