I'll try to help you in English using Google Translate, but you can reply in whichever language you feel comfortable using. Or, if you'd rather have support in Dutch, let us know and we can have them do that. But, keep in mind that you'd have to wait until they're available again.
Now, coming back to your question, are you trying to log in to the account where the form you shared belongs to? If so, the email address on the account is i*fo@he**le**ijn**ad.nl. I assume you're familiar with the email, since it uses the same domain. You will have to log in to Jotform using that email address. If you don't remember the password, and you still have access to its inbox, then you can reset its password. Let me walk you through it:
1. Go to the Jotform Main Page and click on Login.
2. On the login screen, click on Forgot Password.
3. Enter your email address and then click on Send Reset Instructions.
4. You'll get an email with instructions about how to reset your password. In the email, click on Reset Password.
5. Enter your new password, enter it again to verify it, and then click on Create a New Password.
There you go. Your new password is now set, and you can use it to log into to the account.

Just to add up, the title of this thread seems more related to deleting a page, but I checked the form, and I'm not exactly sure what you mean. If what you're trying to do is not about accessing the code, do tell us more details so we can assist you better.
