Hi Tim,
Can you please try to re-integrate your Salesforce Integration into your form? Let me show you how:
- In Form Builder, click Settings on the orange navigation bar.
- On the left side menu, select Integrations then, hover over your mouse and click Salesforce Integration.
- Since you have an existing record that didn't push through to Salesforce, click the Update an existing record before clicking the Save Action button.
