    Now, let me help you with your question. I understand how frustrating this is for you, and I’ll do my best to help you. I checked your account, and it is currently suspended because your form is suspected as a Phishing form.

    To re-activate your account, you need to provide us with the following proof of legitimacy:

    1. The URL where your form is used.
    2. Provide any document that will support your claims.
    3. Your website or proof of affiliation to legitimate institutions.

    Send all of these to reviewers@jotform.com with "Thread #14549481" as the subject. One of our colleagues will get back to you about the status of your account.

    As soon as we hear back from you, we'll be able to help you with this.
