Remove Logo

Ik heb in mijn formulier geen logo ingesteld, maar toch wordt er bij publicatie een logo getoond dat ik niet kan verwijderen blijkbaar. Hoe kan ik dit oplossen?

Well, there is no logo and heading in the form layout, but when published it’s still there…
