フォームに、csvファイルを添付したい。できる方法があれば、教えてください。

サービスの利用申込フォームとして、サービスを有料利用したいと考えています。

フォームの記入者に、利用者情報として「氏名」「メールアドレス」の入力欄を設けています。

フォーム内で直接入力、csvアップロードの2つの方法をフォームに設ける想定です。

そのうちcsvアップロードについて、入力用のフォーマットをフォーム内にテンプレートとして配置し、記入者がダウンロードできるようにしたいです。

上記について、対応方法あれば教えてください。

Regarding your question, I can see two forms in your account. Can you please share the form URL in question? We will also need more information to understand what you are trying to achieve. Do you want the users to upload a CSV file only or do you want to display a CSV file on the form that they can download?
