ログインを行おうとしましたところ、アカウントロックのロックがかかっているという表示が出てしまい、ログインを行うことができません。
こちらの解除方法をご教示いただけますでしょうか。

Unfortunately, our Japanese Support agents are busy helping other Jotform users at the moment. I'll try to help you in English using Google Translate, but you can reply in whichever language you feel comfortable using. If you'd rather have support in Japanese, let us know and we can have them do that.
Now, let me help you with your question. I checked the account associated with this email address, 'shinjuku-west@chm.cigr.co.jp' and it is only linked to the guest account where you also created the form. Please keep in mind that we only provide a one-time login for guest accounts. Do you have another Jotform account we could look into? If not, I recommend creating a Starter (free) account with the same email address you used to create the form. The form created on the guest account will be automatically transferred to your registered account if you use the same email address. Here's the link to the sign-up page.
