フォームが使用できないのと、ログインに問題があるのでご確認いただけますでしょうか

現状フォームにつき、添付のエラーが出てしまって使うことができません。

原因につきましてご確認をお願いできますでしょうか。

また、一昨日よりメールアドレスを使ってのログインをする時に、PWとメールアドレスがマッチしない旨のエラーが出て、ログインができなくなってしまっています。こちらにつきましても不具合かどうかご確認をお願いできますでしょうか。
